Why did I receive a Notice of Fair Fund and Deadline to Submit the Additional Information Request Form and Additional Information Request Form and do I need to do anything in response to that notice?

You received the Notice of Fair Fund and Deadline to Submit the Additional Information Request Form (“Notice”) and Additional Information Request Form because we need additional information from you to determine your eligibility to receive a distribution from the Fair Fund.  Please timely complete and submit the Additional Information Request Form accompanying the Notice, all documentation requested on Additional Information Request Form, and the tax form requested on the bottom of page 2 of the Notice, so that we can accurately determine your eligibility for a distribution.
